We want to inform you that personal health information may have been compromised and we are taking immediate action to protect your privacy. We sincerely apologize for this situation. On February 8, 2012 we discovered we accidentally made personal health information of some of our patients accessible to disclosure by allowing outside search engines to have access to certain files on our internal computer network. The information relates to patients who may have received services from us between February 2011 and August 2011.
The information may include the following: patient name, body mass index (BMI) blood pressure, lab results, smoking status, diagnoses lists, medication allergies, advance directive status, and demographic information which may specifically include spoken language, ethnicity, race, gender, and birth date. We have not identified social security numbers, addresses or financial data were disclosed or made publicly accessible.
The information has been accessible on the Internet since approximately February 2011. We have secured these files within our system and are doing everything possible to eliminate residual or archived information from the Internet.
The information was contained in files that were intended to be maintained securely and used only by us. However, our security settings were incorrect and allowed the data to become publicly accessible on the Internet.
We have partnered with Kroll Information Assurance, Inc., to offer affected patients identity theft and credit monitoring and restoration coverage for one year at no cost. Affected patients will soon be receiving information directly from Kroll. Patients may also call one of the three major credit bureaus listed below and request that a fraud alert be placed on their credit report. At that time, patients may also request a copy of their credit reports.
