Part of the St. Joseph Home Care Network of Northern California, the Sonoma County program is based in Rohnert Park. St. Joseph Home Care provides physical care, emotional care, and spiritual care for patients unable to easily leave their house for medical care. With a personal visit from a healthcare professional, patients of our Home Care Network programs receive support services, medication and symptom management, and medical management for such conditions as AIDS/HIV and diabetes.
