Is your loved one being transferred to one of our hospitals, and you’re
unfamiliar with the area or the next steps that you should take? Our patient
liaison is here to help you with all of your questions and minimize your
stress. The patient liaison can give you answers to questions you have
regarding your or your loved one's care. If the patient liaison does
not have the answers that you’re looking for, they can provide you
contact information for the appropriate department to answer these questions.

can assist you with:
- overcoming any barriers to receiving care
- transportation
- documentation
- financial questions
- overcoming cultural barriers (translation assistance)
- ensuring the patient has all tools needed to make and keep their appointments
